If your record card has "Communication / Documentation" on it, does that mean you didn't turn in a paper or something?

That is just a SAP entry that you had a conversation with your manager. Usually it is for your check-in in your birth month and the 6 month opposite. It can also reflect something less severe than a coaching, like maybe you got called up and reminded to do something in a safer way.

Or you manager could be overboard and just document every time you sit down and talk with them about work related stuff. It varies.
