The train in Mr. Toad, and the boiler room in ToT can really get me if I let myself get lost in the theme.

The boiler room in ToT and when the doors close after the 2 scenes about the "twilight zone" and you know you are about to be dropped. At Disneyland its got to be the snake and/or any skeletons that pop out at you on that ride. The evil queen in Snow White is pretty scary too!

Sometimes on Space Mountain I find myself thinking that there is a broken piece of track hanging down in the dark that is going to hit me in the face and I won't see it coming. Does that count?
The skeleton trying to escape the coffin in the HM and the hallway of knocking doors get to me sometimes, too, but not the same way.
When I was a little kid, still in my stroller for pity's sake, I apparently screamed my head off when I saw the swans at the castle.
Oh the mental image this brought up.

The main one for me is Harold. Not the isolated red eyes in the beginning, but the actual Harold figures. They still get to me. As a kid, I was afraid of almost everything at the park (including IASW and the Fantasyland dark rides).
The seance room in HM gets me because it was my dad's favorite room. Heck, the entire ride gets me because it was his favorite (In a "I miss you" mood, not a AH that's scary one).
The rolling ball that comes towards your vehicle at the end of Indiana Jones. I always have to duck and cover my head every time I see it.
The Matterhorn monster that pops up out of NOWHERE scares the bejsus out of me. I never know what cave it's going to be so I'm always shifty eyed and nervous
