Your AP number listed on your AP login. Just click on "My Profile", and you should see your AP number a little ways down the page. The phone number, at any rate, is not on the AP itself.

Please report the situation immediately to a Main Entrance ticket booth at the Disneyland® Resort, or contact the Annual Passholder Hotline at (714) 781-4546†. You will be required to pay a non-refundable fee of $20.
